All required forms should be completed and submitted on or before E-Day (Everything Day).
E-Day is on Saturday, July 26, 2025.

ALL forms are submitted digitally! You can reduce your wait at E-Day by submitting all of your forms in advance!

Required Forms:
 - Rank One online forms (6 forms in Rank One)
- Student Drug Testing Consent and Enrollment Form
- Medication Administration Permission Form (For all students! Includes OTC meds)
- Custom Meal Request Form (optional)


Rank One Online Forms (online forms):
Online forms must be completed using the Rank One program for all students. Make sure when you are filling out the online forms that you have selected the 2025-2026 school year. There are 6 Rank One forms.

**The physical form is now directly uploaded to Rank One. Students cannot participate in band camp without a completed physical. The physical must be dated after April 1st and be signed and dated by the student, parent, and healthcare provider. It is recommended to keep your paper copy of your physical in case there are any questions.**

COLOR GUARD MEMBERS:

Please choose both Color Guard AND Band when filling out Rank One Forms.

Student Drug Testing Consent & Enrollment (online form):

All LTISD students who participate in extracurricular activities are required to participate in the Student Drug Testing program. This form will be released soon, you will receive an update in CBN.

Medication Administration Form (online form):

This form allows band staff to dispense over-the-counter medication to your student should your student request it. Please complete this form even if you do not want any medication dispensed to your student.

Custom Meal Request Form (optional) (online form):

This is the only optional form. If your student has special dietary needs or allergies, please fill out this form.
